     We look forward to getting to know your child.  Below you will find detailed links to daycare forms that you can look through. All the forms listed are requirements for licensure through the State Of Georgia Department of Early Care and Learning. DAD, Inc. invites you to learn more about us and our mission in becoming your child's educational foundation. DAD  also participates in the USDA funded Food Program from Quality Care for Children. Once enrolled clients will receive more information on this program.

            All application for childcare services requires a copy of child's current  immunization record  and copies of both parents/guardians driver's license to be accepted into facility.
